
Spiritual Care Coordinator Springfield IL Area Part Time
Overview
The Spiritual Care Coordinator reports to the Director Professional Services. Responsibleto provide spiritual support to patients, caregivers and the hospice staff via phone contact, home visits and referrals to the patient/caregiver, local clergy. Assists with memorial services and activities relating to spiritual and religious issues of patient, caregiver and interdisciplinary team.
About You
Bachelor’s degree from an accredited school of theology, counseling, or Social Work or equivalent or ordination in good standing. Two units of Clinical Pastoral Education (CPE) preferred. If affiliated or endorsed by a denomination or faith group must be in good standing with said group, as evidenced by a certificate of ordination or certification.
Ability to develop and maintain rapport with caregiver and patients.
Demonstrated experience in working with patients and caregivers coping with terminal illness and death.
Excellent communication skills, both verbal and written. Excellent interpersonal skills.
Demonstrated problem solving and decision-making abilities.
Knowledge of Hospice philosophy/palliative care and Medicare Hospice benefit rules & regulations.
Experience in patient/caregiver counseling.
Computer skills preferred
Ability to travel 100%
