Afterhours Clinical Support RN

Overview

We are looking for an Afterhours Clinical Support RN (7 Days on-7 Days off) . This position reports to the Manager Afterhours Clinical Support and will provide supportive care to the patient and family after normal business hours on an on-call basis. This position will also manage and direct an interdisciplinary group consisting of Registered Nurses, Licensed Practical/Vocational Nurses, Hospice Aides and support staff to enhance the quality of hospice care.

• Maintains an accurate and updated medical record, including all Interdisciplinary notes, Plans of Care, Initial and on-going assessments and any other information pertinent to the care of the patient.

• Functions as the Administrator on-call throughout the weekend hours

• Performs the initial and on-going assessment of the patient as needed

• Helps develop and implement the Hospice Plan of Care as prescribed by the physician and in conjunction with the other members of the Interdisciplinary Group

About You

• Registered Nurse with minimum of Associate’s Degree in Nursing

• Bachelor’s degree in Nursing (BSN) from an accredited school of nursing preferred

• Current license to practice nursing in the state of work

• Minimum of 1 year experience in hospice

• Minimum of 1 year of management experience preferred
