Overview
We are looking for an Afterhours AIM (Advanced Illness Management) RN to join our team. This position will directly report to the Director of AIM and is responsible foridentifying and coordinating patient/family needs and for providing preventive and supportive care to the AIM patients and family.Utilizes assessment, intervention, and teaching skills to maximize patient comfort and to enhance the quality of life. Assumes afterhours coverage for the designated teams and functions as an interdisciplinary (IDT) team member to provide advanced illness management and support, along with telephonic, video, and in person interventions afterhours.
About You
Current RN Licensure in the state of practice
2 years of prior clinical experience
Experience in palliative care, oncology, hospice, or geriatric care preferred
Must be computer proficient in typing and various programs, including background in EMR
Demonstrated experience developing and maintaining rapport with patients and families, and ability to work well with an interdisciplinary team and PCP
CPR certification, copy of TB results
Current driver license and auto liability coverage
