Overview

We’re looking for a Licensed Practical Nurse Continuous Care to join our team. This position will report directly to the Executive Director/Administrator or Director of Clinical Services, but work under the direction of the Registered Nurse to provide continuous care visits to patients and families in a facility/unit or home setting. The Licensed Practical Nurse Continuous Care will actively monitor patients and report changes in their condition.

  • Achieve palliation and management of acute medical symptoms during brief periods of crisis

  • Under the direction of an RN, provide patient care services to hospice patients and supportive care to the patient’s family with a reasonable case load

  • Evaluate patient/family needs and assist in the preparation and execution of the plan of care for each patient

  • Provide direct patient care, including administering medications and monitoring vital signs

  • Provide education and support to patients and their families regarding their healthcare needs

  • Report findings or changes in the patient’s condition to the Registered Nurse, interdisciplinary team and referring physician and document all reports

About You

  • Graduate of an approved school of Practical Nursing

  • A minimum of two (2) years’ experience as an LPN/LVN, either in long-term care, hospice, or home health.

  • Three to five years’ experience preferred

  • Compassionate, kind and empathetic to our patients and their families

  • Ability to assess and respond to the needs of patients, families and caregivers

  • Certified in CPR
