RN Admissions Hospice Central Intake Team PRN
Overview
Gentiva Home Hospice is looking for an RN Admissions (Central Intake Team) to join our team in Nashville, TN, Region.
This position is PRN/Per Diem status
No "On Call" with the Central Intake positions.

CPR/BLS
Valid Driver's License (TN)
Active Car Insurance
Mileage Reimbursement
Reports directly to the Admissions Manager. You will be responsible for explaining the Company’s services to patients and families in a manner that results in the admission of appropriate patients.
Utilize the nursing process to contribute to each patient’s total plan of care.
Visit patients and families in their place of residence or in the hospital to assess for appropriateness, explain services, monitor condition, or admit to hospice services in accordance with the admission procedure.
About You
You have a Registered Nurse license in the state where the program is located.
Bachelor’s Degree and C.H.P.N. certification preferred.
Minimum of 1 year general nursing experience, Hospice experience preferred.
You have a minimum of 2 years Hospice Health Care or Medical Office experience, preferred.
