
Telephonic Triage Nurse
Overview
We’re looking for a Telephonic Triage Nurse to join our team.You will directly report to the Telephonic Triage Manger. This is a remote position and you will be responsible toassess callers' symptoms, to prioritize calls and to advise patients or families in basic treatment or to make the critical decision to send a nurse to evaluate the patient’s current situation.
Must be fluent in Spanish - The shift is Tues - Wed 5pm - 1am and Sat - Sun 6am - 6pm est.
Receives and responds to calls for assigned Hospice service area
Collects appropriate data in order to make informed decisions related to patients care
Prioritizes various patient care situations, providing information, education, support, and continuity of care in relation to needs identified on the Plan of Care to the patient’s family and/or significant others via the phone or assigns on-call RN or other appropriate discipline to complete an in-person patient visit
About You
Graduate of an accredited school of nursing
Current Registered Nurse (RN) licensure within a NLC Compact State, Three (3) years clinical experience
Must possess a minimum of two (2) years’ experience, either in oncology, long-term care, or hospice or home health
Prior experience with telephone triage
Prior experience using Electronic Medical Records (EMR) and Microsoft Office Suit including Word and Outlook
